Pressured to change how healthcare is paid for, providers and payers are increasingly focusing on population health. According to the University of Wisconsin Population Health Institute, 80 percent of health outcomes are determined by factors outside of clinical care, such as health behaviors, social determinants and the physical environment. What are providers doing to address these factors?  What new partnerships are they forming? What lessons can be learned from community health centers and other groups already deeply engaged in this work? And what’s the role of technology and data in population health?
